Massive Pontine Dissemination of Supratentorial Anaplastic Oligodendroglioma

Abstract

|
Recurrence of anaplastic oligodendroglioma(AO) is well-known complications, even though radical resection, and adjuvant therapy. However, massive pontine dissemination of AO developing from supratentorial portion is an uncommon event. A 27-year-old female presented with a history of generalized tonic-clonic typed seizure and tingling sensation on the right upper extremity. Brain magnetic resonance(MR) images revealed a well-enhanced mass about 3 cm in diameter in the left temporal lobe. She had grossly a total removal of the tumor by craniotomy, and the pathologist reported the final diagnosis as AO. There was no evidence of recurrence on MR images taken at 1 year after the surgery. Three years after surgery, she readmitted due to dysarthria with tingling sensation in the right cheek. She had a recurrent tumor of 5 cm in diameter in the previously operative site. She undertook the second surgery and discharged without any neurological deficits. However, 1 year later, her mental status deteriorated to stupor and brain MR images showed an enlarged tumor in the previously operative area with massive metastasis to the pons. She finally undertook a cyber-knife radiosurgery for a pontine tumor but her mental status remained in stupor. We report a case with massive pontine dissemination of supratentorial AO with review of the pertinent literatures. The authors also emphasize the importance of careful follow-up.
